A DOT physical exam is mandated for commercial motor vehicles or truck drivers in the US to ensure that they are medically, physically, and emotionally fit to drive commercial vehicles safely. Clearing the DOT physical exam is essential to get a CDL. The FMCSA (Federal Motor Carrier Safety Administration) has set some DOT physical regulations and requires only certified medical examiners to conduct the exam.

A DOT physical exam and drug testing is necessary for commercial drivers who:

Operate a commercial vehicle with a gross weight rating of over 10,000 pounds.
Transports more than 15 passengers at a time.
Are paid to operate a commercial vehicle that is designed to carry more than eight passengers.
Transport hazardous materials that require their vehicle to carry a hazmat placard.

Drivers in good standing must achieve:

Vision

You must have 20/40 sharp-sightedness in each eye with or without corrective lenses or contacts and at least 70” peripheral in the horizontal meridian, measured in each eye.

Blood Pressure or Pulse Rate

Your blood pressure should be less than 120/80, and your heartbeat should be between 60 to 100 beats per minute.

Hearing

You should be able to hear a forced whisper within a five-foot distance or less with or without a hearing aid.

Urinalysis

This test is performed to determine whether you have dehydration, diabetes, or any problem in your kidneys, including an infection.

What Must You Bring to a DOT Medical Exam?

Drivers appearing for DOT physicals must bring the following:

Their current medications, including its dosage and schedule.
A letter from their cardiologist (particularly if they have heart problems) to confirm that they are safe enough to drive.
The contact lenses, hearing aids, or glasses that they use.
Recent blood sugar readings if they are diabetic.

Your Physical Exam will include assessment of:
General appearance
Mouth and throat for problems in breathing/swallowing
Lungs and chest for impaired respiratory functions
Heart, to look for a pacemaker, extra sounds, etc.
Eyes for glaucoma, cataracts, macular degeneration, etc.
Abdomen and viscera for enlarged liver and muscle weakness
Genito-urinary, to detect hernias
Neurological system for coordination or speech pattern, impaired equilibrium, and asymmetric deep tendon reflexes
Spine and other musculoskeletal systems for limitation of motion, tenderness, etc.
Ears for perforated eardrums, scarring of the tympanic membrane, etc.
Extremities for impaired limbs
Vascular systems for varicose veins, abnormal pulse, carotid, etc.
